This questionnaire should be completed on your first visit and if applicable, on a weekly basis for returning visitors.

SAFEGUARDING
BEFORE entering School, you are required to Confirm You Understand the Following Protocol:


· Please ensure that you have signed in at the office and sign out before leaving. You MUST bring PHOTO ID.

· If, during your visit, you have any concerns about a child or member of staff please discuss the matter with either Mr Anthony Cadman or any other Deputy/Assistant Head.

· Please do not use your mobile phone or a camera in any area where children are or might be present.

· If you have not had a DBS check you should not be alone with children at any time.

· Never divulge any personal or confidential information.

· Please be aware that certain areas of the school are out of bounds to visitors; for example: the children’s changing rooms, children’s toilets, and the staff room.

· Please follow basic hygiene and health and safety procedures, including fire safety.

· Please ensure the school is aware of any special needs or disabilities that you may have.

FIRE & LOCKDOWN

BEFORE entering School, you are Required to Confirm You Understand the Following Protocol:

If you discover a FIRE

· Sound the alarm

· Do not tackle a fire if you are non-confident in doing so

On Hearing the Alarm

· Please evacuate the building by the safest nearest exit, closing doors behind you.

· Do not stop to collect belongings

· Do not use lifts

· Report to the Assembly point with your host

· Do not return to the building for any reason until authorised to do so

LOCKDOWN

· In the event of a lockdown there will be a continuous sounding School Bell

· Stay out of Sight & Do Not Move

· The All Clear is given by an intermittent sounding of the School Bell

VISITOR ICT ACCEPTABLE USE 
All work undertaken using the Schools Internet and Wi-Fi service is monitored and logged.
Should you wish to use the services you must ensure you are familiar with and understand the Schools Visitor ICT Acceptable Use Policy.  Please follow this link to the policy

SITE HYGIENE INFORMATION

There is no longer a legal requirement in England to self-isolate if you test positive for Covid-19 but please DO NOT enter the school if you have symptoms or are positive.

To help reduce the risk of catching and spreading viruses we ask ALL Visitors to be aware of the following:

 
To Wash or Sanitise their hands regularly throughout their visit.

To Cover cough and sneezes.

Face coverings are no longer compulsory, but visitors can choose to wear a face covering or may be asked to do so at the discretion of the member of staff they are meeting with. 


The school is located in a Radon Gas Affected Area per UK Radon.org, where required access to rooms above the allowed threshold level are locked and have a control notice on the door. Please take note of these.
